Civility and Personal Conduct :
Civil behavior is polite, courteous, and reasonable behavior that is mutually respectful to others and includes integrity, honesty, respect, and effective forms of communication. CAC will not tolerate verbal abuse, including derogatory remarks, insults and epithets, physical conduct of a threat, or intimidation. Such behavior will result in withdrawal from further treatment with our practice. Please continue to be compliant with our policies as we continue to make CAC a safe and comfortable environment.
Late and No Show Policy:
You are required to check in 15 minutes prior to your scheduled appointment time, or you may be asked to reschedule. The check-in time allows our office time to ensure paperwork is up to date, copays and/or outstanding balances can be collected, insurance cards scanned, and any/all questions can be addressed. Please provide our office with 24 hours' notice of cancellation, or you will be charged a $50 fee. A $50 no-show fee will be charged for all missed appointments.
Outstanding Balances/Copays:
Copayments and/or outstanding balances will be due at the time of check-in. You may be asked to show your current insurance card on every visit to ensure accurate claims billing. Per your insurance company, copays and co-insurance amounts are due to the facility as part of the agreement between you and the carrier. These amounts are mandated by your insurance carrier to be collected by the facility. Without the presence of your insurance card and copay, you may be asked to reschedule your appointment.
